Things to Bring to your appointment:

• Your insurance information and card

• Photo identification

You may be asked to bring the following to your appointment:

• Your primary doctor, and/or referring doctor’s name and address

• List of medications and allergies

• Past medical and surgical history records

• Operative reports from previous surgery

• X-ray or diagnostic studies including the actual images and the report from CT scans, MRIs, ultrasounds, nerve tests, etc. 

To cancel your appointment, please let us know 24 hours in advance and we will be happy to reschedule your visit.

Billing and Insurance 

We understand that health care costs are an important and sensitive issue. We are happy to provide cost estimates and discuss any concerns you may have about our billing policies. Certain insurance plans allow us to bill your carrier. In general, payment is expected at the time of service including co-payments, coinsurances, and deductibles. We accept cash, checks and major credit cards.

If you do not have health insurance and are a “self pay” patient, we will provide you with a good faith estimate of charges before you are seen. Please let us know if this is the case.

Requesting a Prescription Refill

To request a refill, you can use MyHealth, ask during office visits or please only call during regular office hours. It is helpful to have your pharmacy’s phone number available when you call.

Emergencies

Dr. Condit or another trusted and qualified physician is available 24 hours a day for emergencies. Call 616-954-1442 to reach our office and someone will answer or return your call as soon as possible. If you need immediate attention, please go to a local emergency room.

Confidentiality

We will not release information about your care without written authorization. You will be asked to review and sign a privacy notice that advises you of our policies in compliance with federal HIPAA confidentiality regulations.
